Left tracheal sleeve intrapericardial pneumonectomy can be performed safely in a single-stage approach incorporating a right posterolateral thoracotomy and left video-assisted thoracoscopy. We present a case that highlights the principles of optimal exposure and adequate ventilation with this single-stage approach.
