Lichen planus pigmentosus inversus after Pfizer-BioNTech COVID-19 vaccination: A rare case report

Abstract:
This study describes a case of a 30-year-old female who developed Lichen Planus Pigmentosus Inversus (LPPI) two weeks after receiving the second dose of the Pfizer-BioNTech COVID-19 vaccine. The patient presented with deep brown macules and patches in the axillae, submammary folds, and groins. Histopathological examination confirmed the diagnosis of LPPI. The patient followed up for two months, during which the patient received topical tacrolimus 0.1 % and corticosteroids, and pigmentation declined within three weeks. No new lesions were observed at the 2-month follow up. This is the fifth worldwide reported case of LPPI post-COVID-19 vaccination, associated with mRNA vaccines. This case highlights the importance of recognizing the potential of LPPI as a rare variant of lichen planus that occurs following Pfizer-BioNTech COVID-19 vaccination. Healthcare providers, especially dermatologists, should maintain awareness of this potential adverse effect to ensure prompt recognition and appropriate management.
