Electrocardiogram Fundamentals01:28

Electrocardiogram Fundamentals

1.4K
Introduction
An electrocardiogram (ECG) is a diagnostic tool for identifying cardiac conditions such as arrhythmias, conduction abnormalities, and myocardial ischemia.
Definition
An electrocardiogram (ECG) visualizes the heart's electrical activity by tracing the electrical movement associated with each heartbeat on a graph or monitor. As the heart beats, an electrical wave passes through it, correlating with the cardiac cycle events.

Electrocardiogram

5.3K
An electrocardiogram (ECG or EKG) is a critical diagnostic tool that records the electrical signals produced by the heart during each heartbeat. This recording is achieved through electrodes placed strategically on the arms, legs, and chest. The electrocardiograph amplifies these signals and produces 12 distinct tracings, offering a comprehensive understanding of the heart's electrical activity.
Three major waveforms are present in a typical ECG recording: the P wave, the QRS complex, and...

Electrophysiology of Normal Cardiac Rhythm

8.7K
The normal cardiac rhythm is a synchronized electrical activity that facilitates the regular and coordinated contraction of the heart muscle. This process is essential for efficient blood circulation throughout the body. The fundamental elements involved in establishing and maintaining this rhythm include the unique electrical properties of cardiac muscle cells, the sinoatrial (SA) node's pacemaker function, the specialized conducting system, and the ionic mechanisms underlying each phase...

Pulse rhythm01:30

Pulse rhythm

1.3K
Pulse rhythm refers to the pattern of pulsations within specific intervals, offering valuable insights into the regularity or irregularity of the heart's beats as observed through the pattern of pulsation within specific intervals. A regular pulse exhibits a consistent heart rate with uniform waveforms and pulsation force, variations of which can be classified as normal, weak, or bounding.
Conversely, an irregular pulse pattern is termed dysrhythmia, stemming from disruptions in cardiac...

Summary

Commanded electrograms (cEGMs) like current of injury and QRS amplitude can predict midterm pacing capture threshold (PCT) after Aveir VR leadless pacemaker implantation. These electrogram parameters can guide fixation strategies for better outcomes.

Area of Science:

  • Cardiology
  • Medical Devices
  • Electrophysiology

Background:

  • Leadless pacemaker implantation using Aveir VR technology relies on commanded electrograms (cEGMs) to assess fixation site suitability.
  • Clinical data on the predictive value and optimal cutoff values of cEGMs for pacing capture threshold (PCT) after implantation are limited.

Purpose of the Study:

  • To investigate the association between midterm pacing capture threshold (PCT) and cEGM parameters following Aveir VR implantation.
  • To determine the predictive value of cEGM parameters for successful pacemaker fixation.

Main Methods:

  • Retrospective analysis of 74 patients undergoing Aveir VR implantation with analyzable cEGM data.
  • Measurement of cEGM parameters (QRS amplitude, duration, current of injury) in premapping and tether modes.
  • Correlation and receiver operating characteristic analyses to identify optimal cutoff values for predicting elevated PCT (>1.5 V/0.4 ms).

Main Results:

  • Current of injury (COI) and QRS amplitude showed significant correlations with PCT.
  • Optimal COI cutoff values were >2.0 mV (AUC 0.668-0.715), associated with higher PCT.
  • Optimal QRS amplitude cutoff values were >5.0 mV (premapping) and >4.0 mV (tether) (AUC 0.771-0.840), strongly predicting elevated PCT.

Conclusions:

  • Pre-fixation cEGM parameters, specifically COI and QRS amplitude, are valuable predictors of midterm PCT after Aveir VR implantation.
  • Utilizing these electrogram parameters can potentially enhance leadless pacemaker fixation strategies and improve procedural success.
