Background:
Carbapenem-resistant Acinetobacter baumannii (CRAB) is a major global threat in healthcare settings and is associated with high morbidity, mortality, and limited treatment options. Effective in-hospital prevention and control of CRAB are critical in curbing its transmission and mitigating the escalating crisis of antimicrobial resistance.
Aim:
We explored enhanced infection-prevention and control (IPC) interventions for reducing CRAB colonization/infection in an intensive care unit (ICU).
Methods:
This retrospective cohort study conducted from 2021 to 2024 was divided into three phases: (1) baseline IPC period (January-December 2021), (2) enhanced IPC intervention period (January 2022-March 2023), incorporating aerosolized hydrogen peroxide (aHP) disinfection and dedicated IPC staff supervision, and (3) follow-up period (April 2023-December 2024). CRAB colonization/infection rates were compared across the phases.
Findings:
Overall, 12,424 ICU patients were analysed (3108 in baseline, 4121 in enhanced IPC, and 5195 in follow-up). The quarterly incidence of CRAB colonization/infection significantly declined from 13.57 cases per 1000 ICU patient-days during baseline to 6.64 after enhanced IPC (P<0.05), with a further reduction to 1.42 in the follow-up period (P<0.05).
Conclusion:
The addition of a touchless aHP enclosed room disinfection system as part of terminal cleaning and supervision by dedicated IPC staff, resulted in a significant and sustained reduction in CRAB colonization/infection rates.
