Adaptive clinical trial designs for rapid vaccine development: Developing country vaccine manufacturers' perspective

Abstract:
Adaptive clinical trial design presents a significant opportunity to accelerate and optimize vaccine development, offering considerable advantages for both National Regulatory Authorities (NRAs) and the pharmaceutical industry, globally as well as in developing countries. By allowing for planned modifications based on interim data, adaptive designs can improve trial efficiency, reduce development timelines, and increase the likelihood of success-all while maintaining scientific integrity and regulatory compliance. For NRAs, adaptive trials can provide earlier insights into safety and efficacy, enabling more responsive oversight and potentially faster access to critical vaccines. They also support data-driven decision-making and pathways for approval, especially during public health emergencies. From an industry perspective, adaptive designs can optimize resource allocation, streamline operations, and enhance the agility of development programs. Despite these benefits, challenges to adaptive design remain especially in low- and middle-income settings. NRAs must possess robust regulatory frameworks and build capacity to evaluate increasingly complex trial methodologies. Industry stakeholders must address statistical, operational, and logistical complexities inherent to adaptive approaches. Transparent communication, early regulatory engagement, and harmonized guidelines are essential to mitigating these challenges associated with adaptive designs. Ultimately, embracing adaptive design requires a shift in clinical development mindset-from rigid, linear models to dynamic, data-informed strategies. It requires early alignment with NRAs on adaptation rules and endpoints, and LMIC ecosystems with dynamic surveillance and and clinical trial sites able to make decisions in real time. As adaptive trial designs become a greater feature in vaccine clinical development, effective vaccines should reach their target populations faster and in a more efficient and economical favorably way ultimately supporting innovation and sustainability of vaccine manufacturers.
