Background:

Intracytoplasmic sperm injection (ICSI) was initially developed for male factor infertility.
Global uptake of ICSI has risen, extending to non-essential fertilization cases.

Purpose of the Study:

To analyze temporal trends in ICSI utilization.
To examine the changing indications for ICSI in Australia.

Main Methods:

A descriptive cohort study of women undergoing IVF in Victoria, Australia (2005-2017).
Analysis of trends in ICSI uptake and reported indications for use.
Inclusion criteria: cycles resulting in a birth after 20 weeks' gestation.

Main Results:

ICSI use increased from 60.6% in 2005 to 79.5% in 2017.
Indications like donor sperm, PGT, female factors, and advanced maternal age increased.
'Unspecified' indications became predominant after 2008.
Male factor infertility indication decreased, while testicular sperm retrieval remained stable.

Conclusions:

Overall ICSI use rose by 18.9% during the study period.
A significant proportion of ICSI cycles were not medically indicated.
The study highlights a trend towards increased, often non-essential, ICSI application.
