Pilonidal sinus disease (PSD) is an acquired pathological illness that typically manifests in the natal cleft of the sacrococcygeal region, but it can rarely occur in other locations, including the penis. We report a 30-year-old circumcised man with a painless, slowly enlarging penile shaft mass. Excision under local anesthesia revealed a hair-containing sinus with chronic granulomatous inflammation on histology, confirming pilonidal disease. Recovery was uneventful. Although uncommon, penile pilonidal sinus should be considered in the differential diagnosis of chronic penile lesions. Histopathology is essential for confirmation, and complete surgical excision provides a definitive cure.
