Summary

Prostate cancer reirradiation often leads to recurrence within the prostate, both inside and outside the high-dose area. Approximately one-third of patients develop regional or distant metastatic disease after treatment.

Background:

  • Local failure of prostate cancer post-radiation therapy indicates a poor prognosis.
  • Previous reirradiation studies have focused on toxicity and oncologic outcomes, with limited data on recurrence patterns.

Purpose of the Study:

  • To investigate the recurrence patterns of prostate cancer following reirradiation.
  • To inform future therapeutic decisions by understanding where recurrences occur after reirradiation.

Main Methods:

  • Retrospective analysis of 33 men with local prostate cancer recurrence after definitive radiation therapy (2012-2018).
  • Reirradiation modalities included focal high dose-rate brachytherapy (23 patients) or stereotactic body radiation therapy (10 patients).
  • Recurrence sites were assessed using multiparametric MRI and compared with radiation dose distribution maps.

Main Results:

  • During a median follow-up of 99 months, 25 patients experienced biochemical recurrence.
  • Of 20 patients with adequate imaging, 5 had recurrences solely within the high-dose region, and 7 had recurrences both inside and outside this region.
  • Four patients presented with both intraprostatic recurrence and regional lymph node metastases; one had prostatic recurrence and bone metastasis.

Conclusions:

  • Prostate cancer frequently recurs within the prostate after reirradiation, affecting areas both inside and outside the high-dose radiation zone.
  • A significant proportion of patients (approximately 1 in 3) develop regional or distant metastatic disease concurrently with or following intraprostatic recurrence.
