Bony Bankart lesions, avulsion fractures of the anterior glenoid rim, pose significant challenges in arthroscopic management. Early surgical management is crucial for restoring shoulder anatomy and mechanics, as delayed or nonsurgical treatment often leads to less favorable outcomes. Various arthroscopic techniques have been described to address bony Bankart lesions, including suture anchor fixation at the glenoid rim, augmentation with additional sutures, and cannulated screw fixation for larger fragments. However, these techniques have limitations, such as a lack of fracture compression, potential fragment tilting, and technical challenges that may result in fragment splitting. The purpose of this Technical Note is to describe a modified arthroscopic bony Bankart auto-bridge repair that offers improved stability, easier suture passage, and the potential for enhanced healing through greater compression and rotational control of the bony lesion.
