Medial Patellofemoral Ligament Reconstruction Using a Trident-Shaped High-Strength Suture Tape-Augmented Hamstring

Abstract:
Medial patellofemoral ligament (MPFL) reconstruction is an established method of treating patellofemoral dislocation. On the basis of the results of recent anatomic studies of the patellar insertion of the MPFL, a trapezoid MPFL can be reconstructed using the autologous semitendinosus tendon. In addition, suture augmentation performed with a suspensory fixation device may help to increase fixation stability. This method may provide more stable patellar fixation with a bioabsorbable suture anchor. Moreover, this method allows anatomic positioning of the graft without extensive bone tunnel creation, which may preserve patellar bone stock. In this technique, the graft assumes a trident configuration-double-bundle hamstring tendon flanking a central high-strength suture tape, reminiscent of a 3-pronged spear. In this technical note, we describe a surgical technique for MPFL reconstruction using ultra-high-molecular-weight polyethylene suture tape-augmented hamstring graft for patellar dislocation.
