Exploring Perineal Reconstruction: Incisional Negative Pressure Wound Therapy for Perineal Flaps-A 2-Case Report

Managing perineal defects presents a clinical challenge requiring a multidisciplinary approach for optimal outcomes. These defects stem from various causes, and their associated complications significantly affect patient morbidity and quality of life. Incisional negative pressure wound therapy (iNPWT) serves as an adaptable adjunct for postreconstruction management across different anatomical areas. We present 2 clinical case reports of patients with perineal defects who underwent comprehensive management, including surgery, antibiotic therapy, and iNPWT. Case 1 involved a 64-year-old woman with advanced neoplastic disease complicated by a rectovaginal fistula. Case 2 featured a 63-year-old man with severe perianal and scrotal infection. Both patients underwent multiple surgical procedures and iNPWT as part of their treatment. Successful wound healing was achieved in both cases, accompanied by resolution of infections and a significant reduction in complications. These cases demonstrate the potential benefits of iNPWT as adjunctive therapy for managing perineal defects. Incorporating iNPWT into the treatment of perineal defects resulting from complex pathologies represents a positive advancement toward enhancing outcomes.
