Summary

A community-based prostate cancer (PCa) education program in Puerto Rico significantly increased knowledge and screening willingness. The intervention successfully detected PCa cases, showing promise for reducing health disparities.

Background:

Prostate cancer (PCa) exhibits high incidence and mortality in Puerto Rico.
Screening and early detection programs for PCa are limited and fragmented.
Focus on rural communities with high African ancestry and elevated PCa mortality.

Purpose of the Study:

Design and implement a community-based educational program to enhance PCa knowledge and screening.
Address barriers to screening in underserved populations.
Evaluate the program's impact on knowledge, confidence, and screening rates.

Main Methods:

Mixed-method study using Community Engagement principles and Intervention Mapping (2021-2025).
Community Advisory Committee involvement in all intervention steps.
Mobile clinic offering PSA testing and digital rectal exams (DRE), with pre/post-tests and satisfaction surveys.

Main Results:

Significant increase in knowledge scores post-intervention (t = -5.5, p < 0.001).
76% of participants reported increased confidence in health decision-making.
95 men screened, 33 referred for follow-up, and 4 PCa cases detected.

Conclusions:

Culturally tailored education combined with accessible screening overcomes barriers.
The program shows potential for reducing PCa disparities in Puerto Rico.
Community-based interventions are effective for improving cancer screening in vulnerable populations.
