Postoperative atrial fibrillation (POAF) increases dementia risk in older adults undergoing noncardiac surgery. Further research is needed to develop preventative strategies for this high-risk group.
Area of Science:
- Geriatric Medicine
- Cardiology
- Neurology
Background:
- Postoperative atrial fibrillation (POAF) is a frequent postsurgical complication linked to increased mortality and morbidity.
- The relationship between POAF and the subsequent development of dementia remains largely unexplored.
Purpose of the Study:
- To investigate the association between POAF and the risk of incident dementia in older adults.
Main Methods:
- Retrospective cohort study utilizing Medicare claims data (2008-2018).
- Included Medicare beneficiaries aged 66+ undergoing surgery, excluding those with prior dementia or stroke diagnoses.
- Primary exposure: atrial fibrillation (AF) status (POAF, no AF, prior AF). Primary outcome: new dementia diagnosis post-hospitalization.
Main Results:
- The study analyzed 670,745 patients; POAF incidence was 22.7% for cardiac surgery and 2.2% for noncardiac surgery.
- Dementia incidence was similar between POAF and no AF groups for cardiac surgery (4.4% vs. 4.6%).
- For noncardiac surgery, dementia incidence was higher in the POAF group (12.5%) compared to the no AF group (9.3%). Adjusted analysis showed an association between POAF and incident dementia in noncardiac surgery (HR: 1.20; 95% CI: 1.11-1.29).
Conclusions:
- Postoperative atrial fibrillation is associated with an increased risk of incident dementia in older adults who undergo noncardiac surgery.
- These findings highlight the need for increased clinical attention and further research into dementia prevention strategies for patients with POAF.
