Abstract:
The recent article by Aoyama and colleagues on recovery of motor and cognitive functions in ICU-acquired weakness provides important insights into patient outcomes following critical illness. Their use of linear mixed modeling and rigorous stratification strengthens the evidence base for understanding cognitive-motor relationships in this vulnerable population. However, several methodological and statistical opportunities merit consideration for future investigations.This letter offers three evidence-based recommendations. First, longitudinal neuropsychological assessments using multidomain instruments (MoCA, MMSE) throughout post-ICU recovery could enhance precision in characterizing cognitive trajectories beyond single time-point FIM scores. Second, advanced statistical techniques such as propensity score matching or inverse probability weighting may better address baseline heterogeneity and residual confounding inherent in observational studies. Third, detailed documentation of post ICU rehabilitation protocols, including frequency, intensity, and patient adherence, using standardized reporting frameworks would clarify the contribution of specific interventions to both cognitive and motor recovery.These enhancements would strengthen future research in ICU-acquired weakness, ultimately optimizing patient recovery and informing clinical practice standards.
