    Purpose of the Study:

    • To enhance patient understanding of newly prescribed cardiac medications at discharge.
    • To increase the percentage of patients understanding medication indications and side effects to at least 85% by January 2024.
    • Utilized the Plan-Do-Study-Act framework for a quality improvement initiative.

    Main Methods:

    • Assessed 50 eligible patients (mean age 71.7) on medication knowledge via questionnaire.
    • Provided tailored education using plain language and teach-back for patients with knowledge gaps.
    • Conducted follow-up interviews 2-4 weeks post-discharge to assess information retention.

    Main Results:

    • Baseline understanding of medication purpose was 54%, and of side effects was 26%.
    • Post-intervention, 92% accurately explained medication indications and side effects.
    • 74% retained this information at follow-up, indicating sustained comprehension.

    Conclusions:

    • Teach-back significantly improved patient comprehension and short-term retention of discharge medication instructions.
    • This method is a feasible, low-cost strategy for inpatient cardiology settings.
    • Teach-back effectively addresses health literacy-related communication gaps and supports safer care transitions.
