Background:

Intestinal ultrasonography (IUS) is a cost-effective tool for diagnosing and monitoring IBD.
Widespread adoption of IUS is hindered by knowledge gaps in evidence, training, and standardization.

Purpose of the Study:

To identify critical knowledge gaps in IUS research for IBD.
Focus areas include disease activity, postoperative recurrence, complications, pediatric use, contrast enhancement, elastography, and AI integration.

Main Methods:

A systematic umbrella review following PRISMA guidelines was conducted.
Searched MEDLINE for systematic reviews, meta-analyses, and consensus statements up to February 2025.
Identified and categorized research gaps based on evidence quality and data availability.

Main Results:

Sixty studies revealed significant gaps in validated IUS indices for Crohn's disease and ulcerative colitis.
Limited evidence exists for IUS in postoperative recurrence, pediatrics, and pouch disease.
Data on contrast-enhanced ultrasound, elastography, training, and AI applications are sparse.

Conclusions:

Substantial evidence gaps remain for IUS in IBD management.
Multicenter studies and consensus frameworks are crucial to enhance IUS clinical and research utility.
