Direct retrieval of posteriorly dislocated intraocular lenses via limbal air bubble technique

Purpose:
To introduce a limbal-based technique for retrieving posteriorly dislocated IOLs without pars plana vitrectomy, utilizing an anterior chamber air bubble for fundus visualization.
Methods:
In this retrospective study, 3 limbal paracentesis incisions (2, 6, and 10 o'clock) were created in 9 eyes. An air bubble was injected into the anterior chamber to enable direct fundus visualization. The dislocated IOL was grasped via a forceps inserted through limbal incisions and repositioned or exchanged without pars plana vitrectomy.
Results:
9 patients (9 eyes) with posteriorly dislocated IOLs were enrolled in this retrospective study. In all cases, the dislocated IOLs were successfully elevated to the anterior segment. Among them, 7 IOLs were rescued and 2 were exchanged. No complications were observed, except for temporary high intraocular pressure in two cases immediately after the operation. No IOL redislocation was observed during a mean follow-up of 12 months.
Conclusion:
Retrieving a posteriorly dislocated IOL through a limbal approach utilizing an anterior chamber air bubble is an efficient, safe, cost-effective, and easily accessible technique.
