Nonsense-mediated mRNA Decay02:27

Nonsense-mediated mRNA Decay

11.6K
The Upf proteins that carry out nonsense-mediated decay (NMD) are found in all eukaryotic organisms, including humans. Each protein has an individual role, but they need to work in collaboration. Upf1 is an ATP-dependent RNA helicase that unwinds the RNA helix. Because Upf1 can unwind any RNA, Upf2 and Upf3 are required to help Upf1 discriminate between nonsense and normal mRNAs.
Usually, Upf3 binds to an Exon Junction Complex (EJC) at mRNA splice sites. If a ribosome fully translates the mRNA,...

Spontaneity02:21

Spontaneity

28.5K
A spontaneous process is one that occurs naturally under certain conditions. A nonspontaneous process, on the other hand, will not take place unless it is “driven” by the continual input of energy from an external source. Processes have a natural tendency to occur in one direction under a given set of conditions. Water will naturally flow downhill (spontaneous process), but uphill flow (nonspontaneous process) requires outside intervention such as the use of a pump. Iron exposed to...

Ellipses01:30

Ellipses

176
An ellipse is formed when a right circular cone is intersected by an inclined plane that does not cut through its base. This intersection yields a closed, symmetric curve characterized by distinctive geometric properties. Most notably, an ellipse is defined as the collection of all points in a plane for which the combined distances to two fixed points—called the foci—remain constant. The ellipse features two principal axes: the major and the minor axes. The major axis is the longest...

Counterfactual Thinking01:19

Counterfactual Thinking

194
Counterfactual thinking is a cognitive process wherein individuals mentally reconstruct alternative versions of past events, often beginning with “what if” or “if only.” This reflective mechanism plays a significant role in shaping emotional experiences and guiding future behavior. Though typically triggered by unfavorable or unexpected outcomes, counterfactual thinking can also emerge in mundane, everyday decisions and experiences, revealing its deep entrenchment in...

Schemas01:42

Schemas

12.3K
A schema is a mental construct consisting of a cluster or collection of related concepts (Bartlett, 1932). There are many different types of schemata, and they all have one thing in common: schemata are a method of organizing information that allows the brain to work more efficiently. When a schema is activated, the brain makes immediate assumptions about the person or object being observed.

Summary

A brain lesion mimicking a tumor caused a patient to speak nonsensically. Anti-convulsive therapy resolved the conduction aphasia, suggesting focal epilepsy.

Area of Science:

  • Neurology
  • Neuroscience
  • Neurosurgery

Background:

  • Wernicke's area is crucial for language comprehension.
  • Conduction aphasia presents with fluent but nonsensical speech and impaired repetition.
  • Intracranial neoplasms can manifest with diverse neurological symptoms.

Purpose of the Study:

  • To report a case of conduction aphasia secondary to a suspected intracranial neoplasm.
  • To highlight the diagnostic challenge posed by focal epilepsy mimicking a tumor.
  • To emphasize the importance of considering epilepsy in the differential diagnosis of acute aphasia.

Main Methods:

  • Case presentation of a 61-year-old male with acute onset of fluent, nonsensical speech.
  • Magnetic resonance imaging (MRI) revealing a contrast-enhancing lesion near Wernicke's area.

  • Clinical assessment and response to anti-convulsive therapy.
  • Main Results:

    • The patient exhibited fluent aphasia with preserved comprehension.
    • Imaging identified a lesion suggestive of a neoplasm, ruling out stroke or hemorrhage.
    • Symptoms resolved completely following initiation of anti-convulsive medication.

    Conclusions:

    • Focal epilepsy can present with symptoms mimicking an intracranial neoplasm, specifically conduction aphasia.
    • Prompt diagnosis and treatment of epilepsy are essential for symptom resolution.
    • This case underscores the need for a broad differential diagnosis in acute neurological presentations.
