Mitral Jamming by Left Atrial Mobile Thrombus in Severe Rheumatic Mitral Stenosis

Background:
Left atrial (LA) thrombus occurs in 17% of patients with severe mitral stenosis (MS), and the incidence doubles if it is accompanied by atrial fibrillation. Severe MS and large LA mobile thrombus (LAMT) pose a significant risk of hemodynamic collapse.
Case Summary:
A 55-year-old woman with LAMT obstructing the left ventricular inflow tract in severe MS presented with hemodynamic deterioration. Urgent mitral valve replacement (MVR) and thrombectomy were performed. The patient was discharged on day 39.
Discussion:
Urgent MVR and LA thrombectomy are essential in ball-valve obstruction by thrombus. In this case, prompt referral and performing an emergent surgical approach led to a favorable postoperative outcome.
Take-Home Messages:
A large LAMT in severe MS is a life-threatening condition when the thrombus obstructs the stenotic mitral valve. Prompt referral and performing urgent surgical MVR and thrombectomy could be a life-saving measure in this clinical condition.
