Summary

Exercise adherence and compliance rates in progressive multiple sclerosis (PMS) were examined. Compliance was lower than adherence, especially for high-intensity interval training (HIIT), with physical and cognitive function influencing outcomes.

  • Understanding exercise training (ET) adherence and compliance in progressive multiple sclerosis (PMS) is limited.
  • This study addresses the gap in knowledge regarding ET effectiveness in PMS populations.

Purpose of the Study:

  • To analyze adherence and compliance rates for continuous, moderate-intensity training and high-intensity interval training (HIIT) in people with PMS.
  • To identify potential correlates of adherence and compliance within the CogEx trial.

Main Methods:

  • A secondary analysis of the multi-site, randomized, double-blinded CogEx trial involving 311 participants.
  • Focus on 152 participants who received and attempted ET interventions (continuous or HIIT) over 12 weeks.

Main Results:

  • Overall adherence was 94% and compliance was 66%.
  • Continuous exercise showed 95% adherence and 73% compliance; HIIT showed 92% adherence and 58% compliance.
  • Better 6-minute walk test (6MWT) performance correlated with higher compliance (especially for HIIT), while better CVLT-II performance correlated with higher adherence and compliance for continuous training.

Conclusions:

  • Compliance with ET, particularly HIIT, is lower than adherence in people with PMS.
  • Cognitive and physical function are key correlates influencing adherence and compliance in ET for PMS, informing future trial designs.
