Percutaneous Endoscopic Gastropexy for Gastric Volvulus in a High-Risk Older Adult Patient

Gastric volvulus is a rare condition that may recur after endoscopic reduction. Although surgical fixation is often performed to prevent recurrence, this may be difficult in older adults with poor operative tolerance, necessitating alternative approaches. Here, we report a case of gastric volvulus without hiatal hernia in a 78-year-old woman that was successfully managed with percutaneous endoscopic gastropexy after endoscopic reduction. Four-point fixation was performed using devices commonly employed for gastric wall anchoring during percutaneous endoscopic gastrostomy. No recurrence was observed. This case highlights endoscopic gastropexy as an effective, minimally invasive option for preventing gastric volvulus recurrence in high-risk patients.
