Motor complications and postural abnormalities interplay in Parkinson's disease

Postural abnormalities in Parkinson's disease are linked to more severe motor complications and non-motor symptoms. However, they do not predict the future development of these motor complications over time.

Background:
- Postural abnormalities (PA) and motor complications (MCs) are key features of Parkinson's disease (PD) progression.
- The relationship between PA and MCs, such as motor fluctuations (MFs) and levodopa-induced dyskinesia (LIDs), is not well understood.
Purpose of the Study:
- To examine the association between PA and MCs, motor symptoms, and non-motor symptoms (NMS) in PD patients.
- To determine if PA influences the future development of MCs.
Main Methods:
- Analysis of data from the prospective NS-Park cohort across 27 French PD Expert Centers.
- PA defined by a score ≥2 on MDS-UPDRS-III item 3.13. Logistic regression and interval censoring survival models were used.
- Adjustments made for sex, age, disease duration, dopaminergic dose, and disease severity.
Main Results:
- Out of 13,037 PD patients, 5.6% had PA. Patients with PA had longer disease duration, higher severity, and higher dopaminergic treatment.
- PA was associated with a higher prevalence of troublesome MFs (OR: 5.96) and LIDs (OR: 2.81).
- No significant association was found between PA and the development of MCs during follow-up.
Conclusions:
- Postural abnormalities are linked to more frequent and severe motor complications in Parkinson's disease.
- PA is associated with a greater burden of motor and non-motor symptoms, complicating patient care.
- PA does not appear to predict the future incidence of motor complications.
