Risk Factor of Postoperative Complications Following Pneumonectomy for Destroyed Lung

Introduction:
The incidence of complications following pneumonectomy for destroyed lung (DL) is notably high. This study aims to identify the risk factors associated with secondary complications after DL pneumonectomy.
Materials And Methods:
A retrospective analysis was performed on the clinical data of patients who underwent left or right lung resection due to DL at our hospital from November 2002 to January 2024. Postoperative complications included death within 30 d after surgery, respiratory failure, and pulmonary infection, among others. The study included 49 cases in the postoperative secondary complication group. Clinical characteristics and outcomes were compared between the complication and noncomplication groups to analyze the risk factors for complications after DL surgery.
Results:
Postoperative complications were observed in 49 cases (36.3%). The complication group exhibited older age, a higher prevalence of chronic pulmonary aspergillosis and concomitant hemoptysis, lower serum albumin levels, and increased intraoperative blood loss compared to the noncomplication group.
Conclusions:
The mortality rate following DL pneumonectomy is relatively high. Preoperative management of hemoptysis and drug resistance, along with strategies to reduce intraoperative blood loss, is crucial in minimizing postoperative complications.
