Chronic total occlusion (CTO) percutaneous coronary intervention (PCI) represents one of the most technically challenging procedures in interventional cardiology, with success rates approaching 90% but higher complication rates than non-CTO PCI. This review provides systematic approaches to managing major CTO PCI complications including coronary perforation (3.8%), donor vessel injury (0.35%), and equipment entrapment (0.4%). Key management principles emphasize early recognition through systematic assessment, immediate intervention with appropriate techniques, and knowing when to stop procedures. Successful complication management requires readily available emergency equipment, trained staff, and sound clinical judgment to optimize patient safety during these complex interventions.
