Abstract:
Even in the most experienced centers, when chronic total occlusion (CTO) percutaneous coronary intervention (PCI) is performed in patients, the success rate remains around 85% to 90%. Subintimal tracking and reentry and subintimal plaque modification (SPM) have emerged as a potential solution for these challenging cases. While subintimal tracking and reentry offers a high success rate with a low complication rate, the optimal timing and management of balloon angioplasty and stent implantation remains a debate. This article will cover the subintimal tracking and reentry and the SPM techniques, how to perform them, and data surrounding their outcomes.
