Background:

  • Intermittent exotropia (IXT) is a common strabismus with varied clinical presentations.
  • Understanding subtype-specific characteristics is crucial for effective management.

Purpose of the Study:

  • To classify intermittent exotropia into distinct types based on distance and near exodeviation angles.
  • To delineate the unique clinical features associated with each subtype.

Main Methods:

  • A cohort of 5331 intermittent exotropia patients was analyzed.
  • Patients were categorized into Basic-type, Convergence Insufficiency (CI)-type, and Divergence Excess (DE)-type based on exodeviation differences.
  • Demographics, clinical characteristics, symptoms, and medical/family history were assessed.

Main Results:

  • Basic-type IXT comprised 86.2%, CI-type 9.4%, and DE-type 4.4% of the cohort.
  • CI-type IXT correlated with older age and higher myopia, often presenting with A-pattern, superior oblique overaction, good fusion, stereoacuity, and diplopia.
  • DE-type IXT was associated with superior oblique underaction and photophobia.

Conclusions:

  • Distinct clinical profiles exist for Basic, CI-type, and DE-type intermittent exotropia.
  • CI-type IXT is characterized by older age, myopia, and specific ocular findings.
  • DE-type IXT shows a predilection for photophobia.
