Fractures: Bone Repair

Treatment for a fracture is based on the type of break, the bone affected, and the patient's age.
Minor fractures with no bone displacement are treated by immobilizing the fractured bone using a cast or splint. However, in the case of fractures with displaced bones, the broken bones are repositioned before immobilization to ensure successful healing without deformation and loss of function. The realignment of fractured bone ends is performed through a process called reduction. If the procedure...

Summary

Titanium miniplate fracture after mandibular condylar fracture fixation is rare. Metal fatigue caused by bruxism and smoking contributed to this rare complication in a patient with a condylar neck fracture.

Area of Science:

  • Oral and Maxillofacial Surgery
  • Biomaterials Science
  • Orthopedic Surgery

Background:

  • Fracture of titanium miniplates used for internal fixation of mandibular condylar fractures is an uncommon complication.
  • Previous studies indicate a low incidence rate of approximately 2.6%.

Purpose of the Study:

  • To report a rare case of titanium miniplate fracture after internal fixation of a mandibular condylar fracture.
  • To investigate the potential causes of miniplate failure in this specific case.

Main Methods:

  • A case report of a 54-year-old female patient with a right condylar neck fracture treated with titanium miniplates.
  • Postoperative radiographic evaluation and scanning electron microscopy (SEM) of the fractured miniplate.
  • Patient history including diet, smoking, and parafunctional habits like bruxism.

Main Results:

  • A fracture of a titanium miniplate was identified 5 months postoperatively.
  • SEM analysis revealed striation lines indicative of a metal fatigue fracture.
  • Patient-reported bruxism and non-adherence to smoking cessation were identified as contributing factors.

Conclusions:

  • Metal fatigue is a likely cause of titanium miniplate fracture in mandibular condylar fracture fixation, particularly when combined with patient-related factors such as bruxism and smoking.
  • Adherence to postoperative protocols and management of parafunctional habits are crucial for long-term implant survival.
