Background:

  • Endolymphatic hydrops (EH) is increasingly identified via Hydrops MRI in patients with profound sensorineural hearing loss (SNHL).
  • This finding presents a discrepancy requiring investigation into the underlying mechanisms of SNHL in these cases.

Purpose of the Study:

  • To investigate the mechanisms behind profound SNHL in patients diagnosed with endolymphatic hydrops (EH).

Main Methods:

  • Evaluated 184 patients with Hydrops MRI, identifying 6 (3%) with profound SNHL.
  • Conducted comprehensive inner ear testing followed by Hydrops MRI for all affected patients.
  • Assessed the prevalence of EH in the cochlea and vestibule.

Main Results:

  • EH was present in the cochlea (67%) and vestibule (42%) of affected ears.
  • 50% of patients experienced rapid SNHL onset (1-2 months) in both ears, linked to superimposed infections.
  • The other 50% showed a delayed SNHL onset (5-8 years), attributed to the progression of EH.

Conclusions:

  • Profound SNHL in EH ears suggests two pathways: superimposed infection on pre-existing hydrops or delayed EH development.
  • Rapid onset implies infection exacerbating existing hydrops, while gradual onset indicates a slower progression of EH.
