Back to the future: moving beyond a 1948 mindset to design NHS dentistry for 2025

NHS (National Health Service) dentistry began in 1948 as a cornerstone of Britain's commitment to universal healthcare. At that time, the focus was clear and necessary: to address the overwhelming backlog of unmet need. Within the first year, more than 33 million teeth were extracted and 4.5 million dentures provided. Dentistry's defining contribution to the early NHS was rapid relief from pain and restoring function for a war-torn population. But that was 1948. The context in 2025 could not be more different - scientifically, economically, socially, and professionally. And yet, the policy mindset shaping NHS dentistry remains stubbornly rooted in the past. Government strategy continues to focus on widening access without adequately addressing what kind of care is being accessed, who needs it most, and how we can afford to provide it sustainably. This article argues that we must move beyond a 1948 model of universalism and instead design a targeted, quality-focused, and future-proof NHS dental service. That means prioritising prevention, emergency care, and comprehensive support for the most vulnerable, rather than chasing an unaffordable 'gold standard' for all.
