Understanding Glycemia in the Post Discharge Period Through Blinded Continuous Glucose Monitoring

Post-hospital discharge glucose control for diabetes patients is often suboptimal. Continuous glucose monitoring (CGM) can identify hypoglycemia and inpatient glucose levels may predict post-discharge glycemic control.
