Neurogenic paraosteoarthropathy: a case report of bilateral knee involvement

Neurogenic paraosteoarthropathy (NPOA) is a rare complication of severe neurological trauma, characterized by heterotopic bone formation in periarticular soft tissues. We report the case of a 35-year-old man who developed bilateral knee stiffness after traumatic brain injury (TBI). Computed tomography (CT) demonstrated large para-articular ossifications without joint involvement. Laboratory results excluded metabolic or infectious causes. The patient was treated conservatively with physiotherapy and nonsteroidal anti-inflammatory drugs, with partial improvement at 1-year follow-up. This case highlights the importance of CT in diagnosis and management of NPOA, especially in unusual localizations such as the knees.
