Summary

Most soft tissue lumps assessed as low-risk are benign and can be managed locally. Clinical assessment and ultrasonography effectively identify low-risk lesions, reducing unnecessary advanced imaging like MRI.

Background:

  • Current sarcoma guidelines recommend magnetic resonance imaging (MRI) and specialist referral for soft tissue lumps ≥5 cm, or those that are enlarging, painful, or deep.
  • Secondary hospitals frequently manage subcutaneous lesions ≥5 cm without high-risk features, potentially leading to resource overutilization.

Purpose of the Study:

  • To evaluate the efficacy of current guidelines in managing soft tissue lesions in secondary hospitals.
  • To determine if clinical assessment and ultrasonography can adequately identify low-risk lesions, thereby optimizing resource allocation.

Main Methods:

  • A retrospective cohort study was conducted on 552 patients investigated for subcutaneous, soft tissue lesions at a single center.
  • Data were collected from November 2015 to September 2024, analyzing lesion characteristics, imaging modalities, and histopathological outcomes.

Main Results:

  • Of 461 low-risk lesions excised locally, 99.8% were benign, with lipomas being the most common (83.5%).
  • Only one malignant lesion (<5 cm) was found among the low-risk excised lesions.
  • Pre-operative ultrasound demonstrated high sensitivity (95.3%) and specificity (100%) for lipomas, while MRI was used less frequently (19.1%).

Conclusions:

  • Size alone is not a definitive indicator for escalating investigations like MRI.
  • Clinical assessment combined with ultrasonography is sufficient for identifying low-risk soft tissue lesions suitable for local excision in non-sarcoma centers.
  • Current practices may lead to overutilization of resources; a refined approach focusing on clinical and ultrasound findings can improve efficiency.
