Background:

  • Takotsubo cardiomyopathy is a critical condition characterized by acute left ventricular dysfunction.
  • Myocardial recovery in takotsubo cardiomyopathy patients may be improved by physical exercise or cognitive behavioral therapy.

Purpose of the Study:

  • To investigate the efficacy of physical exercise training and cognitive behavioral therapy in enhancing myocardial recovery post-takotsubo cardiomyopathy.
  • To compare the effects of these interventions against standard care on cardiac energetics and exercise capacity.

Main Methods:

  • A prospective, multicenter clinical trial randomized 76 patients with acute takotsubo cardiomyopathy into three groups: physical exercise training, cognitive behavioral therapy, or standard care for 12 weeks.
  • The primary endpoint was the resting phosphocreatine/gamma-ATP ratio measured by 31P-magnetic resonance spectroscopy.
  • Secondary endpoints included peak exercise oxygen consumption, 6-minute walk distance, left ventricular global longitudinal strain, and heart failure symptom burden.

Main Results:

  • Both physical exercise training and cognitive behavioral therapy significantly improved the myocardial phosphocreatine/gamma-ATP ratio compared to standard care (P=0.016 and P=0.043, respectively).
  • Exercise capacity, measured by peak oxygen consumption and 6-minute walk distance, was also significantly enhanced in both intervention groups versus standard care (P<0.05 for all).
  • No significant differences were observed in left ventricular global longitudinal strain or symptom burden between the groups.

Conclusions:

  • A 12-week program of physical exercise or cognitive behavioral therapy effectively improves left ventricular myocardial energetics and exercise performance in patients recovering from takotsubo cardiomyopathy.
  • These non-pharmacological interventions show promise for enhancing recovery but do not appear to directly alleviate heart failure symptoms in the short term.
