Complex Outflow Graft Thrombosis of a Patient on Durable Left Ventricular Assist Device: An Interventional Treatment

Case Summary:
A 54-year-old female patient on long-term left ventricular assist device (LVAD) support presented with acute onset of low cardiac output due to LVAD low-flow, based on a complex thrombus formation extending directly to the pump. Interventional treatment, including precautions to prevent embolization, aspiration of floating thrombus formations, and implantation of an expandable endoprosthesis, led to a complete normalization of the LVAD flow. Twelve months after the procedure, we see no signs of recurrent thrombosis.
Take-Home Message:
Interventional thrombus aspiration and endovascular stenting, combined with embolism prevention, can be a safe and less-invasive treatment option for patients with outflow graft thrombosis.
