    Background:

    • Psoriatic Arthritis (PsA) is a complex chronic inflammatory condition impacting quality of life.
    • Current PsA diagnosis and monitoring rely on subjective assessments by healthcare professionals (HCPs).
    • Objective, scalable tools are needed for PsA management.

    Purpose of the Study:

    • To introduce a video-based digital biomarker (dBM) approach for assessing hand joint mobility in PsA patients.
    • To develop and validate the HD4PsA smartphone application for capturing hand motion data.
    • To evaluate the clinical relevance of dBMs derived from smartphone video analysis.

    Main Methods:

    • Development of the HD4PsA smartphone app with three hand motion tests (hook, full fist, wrist flexion/extension).
    • Acquisition of hand joint coordinate data via smartphone video during motor tests.
    • Processing of landmark data to generate hand joint mobility dBMs.
    • Validation using data from 16 PsA patients and 9 healthy controls (HC).

    Main Results:

    • A subset of proposed dBMs showed statistically significant differences between PsA patients and HC.
    • These dBMs demonstrated sufficient performance as binary classifiers for PsA detection.
    • A fair correlation was observed between limited range of motion (ROM) and PsA inflammation grade.

    Conclusions:

    • The HD4PsA app and its derived hand joint mobility dBMs show potential for clinical application in PsA.
    • This digital biomarker approach offers objective, patient-centric assessment of PsA.
    • Smartphone-based dBMs can provide reliable indicators for PsA diagnosis and monitoring.
