Day and Night Patterns of Heart Rate Variability in Type 2 Diabetes: Gender and Microvascular Complications
Abstract:
Reduced heart rate variability (HRV) is associated with metabolic syndrome and has proven useful for assessing autonomic nervous system (ANS) function and its alterations in individuals with diabetes mellitus. This study investigates the patterns of HRV features recorded in people with type 2 diabetes (T2DM), considering day and night differences across albuminuria categories, with gender and microvascular complications lens. HRV metrics were extracted and analysed from 24-hour Holter ECG recordings in a cohort of individuals diagnosed with T2DM for over 10 years at Bangladesh Institute of Health's Hospital. HRV patterns are more evident at night, in normal albuminuria and in individuals with microvascular complications. This is the first study that has investigated the relationship of HRV in people with diabetes and their albuminuria condition and the influence of circadian rhythms on HRV. Our finding has the potential for ECG-based detection and monitoring of microalbuminuria in T2DM without wet lab tests.Clinical Relevance - Understanding these patterns may enhance predictive models for the progression of diabetic kidney disease.
