Abstract:
Potassium competitive acid blockers (P-CABs), such as vonoprazan, fexuprazan, tegoprazan and revaprazan are an innovative class of drugs designed to treat conditions related to gastric acid secretion, including gastroesophageal reflux disease (GERD), Helicobacter pylori (Hp) infection and peptic ulcer disease. These drugs work by reversibly and competitively inhibiting the H+/K+-ATPase proton pump, which allows them to suppress gastric acid production more effectively and faster than traditional proton pump inhibitors (PPIs). In addition, these drugs are not affected by the CYP2C19 enzyme and their effect is independent of food intake, giving them advantages over most PPIs. Clinical studies have shown that P-CABs are superior to PPIs in the treating of severe erosive GERD and non-inferior in mild and non-erosive disease. In addition, P-CABs offer non-inferior and in some cases superior Hp eradication rates to PPIs in first-line treatment, but not in second-line treatment. In conclusion, P-CABs are emerging as promising alternatives in the treatment of acid-dependent gastrointestinal disorders.
