Area of Science:

  • Integrative Medicine
  • Dermatology
  • Clinical Research

Background:

  • Neurodermatitis is a chronic inflammatory skin condition impacting quality of life.
  • Current treatments offer variable efficacy and potential side effects.
  • Fire needling therapy is an emerging complementary approach.

Purpose of the Study:

  • To systematically evaluate the clinical effectiveness and safety of fire needling therapy for neurodermatitis.
  • To compare fire needling as monotherapy versus combination therapy with conventional treatments.

Main Methods:

  • A systematic review and meta-analysis of randomized controlled trials (RCTs) following PRISMA 2020 guidelines.
  • Searched Chinese and English databases up to October 2024, including 10 RCTs with 868 patients.
  • Assessed outcomes such as effectiveness, recurrence, pruritus, inflammatory markers, DLQI, and adverse events.

Main Results:

  • Fire needling monotherapy did not significantly outperform conventional treatments.
  • Combined therapy showed significantly higher effectiveness rates (>90% after 4 weeks) and reduced recurrence rates within 6 months.
  • Combined therapy improved Dermatology Life Quality Index (DLQI) scores, reduced pruritus, and modulated key inflammatory markers (TNF-α, IL-4, IL-6, IL-8, IgE).
  • Adverse events associated with fire needling were reported as mild.

Conclusions:

  • Fire needling therapy, particularly in combination with conventional treatments, shows promise for enhancing neurodermatitis management.
  • This approach may lead to reduced pruritus, improved quality of life, and better control of inflammation.
  • Further high-quality RCTs are recommended to confirm findings due to study limitations and potential publication bias.
