Some physical activities temporarily increase low back pain (LBP) flare risk but do not impact long-term function. Engaging in more activities did not lead to better or worse long-term outcomes for individuals with LBP.

Background:

  • Physical activity presents a complex relationship with low back pain (LBP), offering both potential harm and benefit.
  • Understanding the short-term and long-term effects of specific activities is crucial for managing LBP.

Purpose of the Study:

  • To assess the transient (short-term) risk of 10 common physical activities on LBP exacerbations (flares).
  • To evaluate the cumulative (long-term) risk of these activities on LBP-related functional limitations over a 1-year period.

Main Methods:

  • A prospective, longitudinal case-crossover study nested within a cohort.
  • Recruited adults with recent healthcare visits for LBP.
  • Followed participants for 1 year, collecting data on physical activity exposure and LBP outcomes.

Main Results:

  • Increased hourly engagement in lifting, bending, pushing/pulling, twisting, and squatting was associated with a higher risk of subsequent LBP flares.
  • Prolonged sitting was associated with a reduced risk of LBP flares.
  • No association was found between mean hours spent in activities during the initial 8 weeks and functional limitations at 1-year follow-up.

Conclusions:

  • Certain physical activities pose short-term, transient risks for LBP flares.
  • No evidence suggests these activities influence long-term functional limitations in individuals with LBP.
  • Engaging in these activities does not appear to alter long-term LBP-related functional outcomes.
