Case:
The optimal treatment of large, medial Hill-Sachs lesions is unclear and continues to be investigated. Remplissage has shown promising results but can be less advantageous with medial, deep, or poor bone quality lesions and revisions. Current open techniques require extensile approaches with partial or complete subscapularis violation.
Conclusion:
This cases describes an open posterolateral deltoid-splitting approach with circular fresh osteochondral allograft implantation for large off-track Hill-Sachs lesions.
