Background:
Patients presenting with myocardial infarction and coronary artery ectasia (CAE) face an increased risk of recurrent cardiovascular events. We present a case of unrecognized CAE contributing to stent failure.
Case Summary:
A 52-year-old male presented with an inferior ST-segment elevation myocardial infarction and underwent percutaneous coronary intervention to the right coronary artery (RCA). Fifteen months later, he re-presented with another inferior ST-segment elevation myocardial infarction due to stent occlusion, but percutaneous coronary intervention was unsuccessful. Two weeks later, research-initiated computed coronary tomography angiography revealed a previously unrecognized large, ectatic, thrombosed RCA with unopposed stents. This prompted review of the invasive angiography leading to new appreciation of RCA calcification, supportive of CAE.
Discussion:
CAE poses challenges in acute myocardial infarction management, increasing interventional complexity and potential need for antithrombotic therapy.

While ectasia may not always be appreciated on fluoroscopic imaging, its recognition is vital for guiding revascularization and secondary prevention strategies.
