Multimodal imaging predictors of recurrence after photodynamic therapy in chronic central serous chorioretinopathy

Purpose:
To evaluate the correlation between spectral-domain optical coherence tomography (SD-OCT), fundus fluorescein angiography (FFA), and indocyanine green angiography (ICGA) findings and recurrence following photodynamic therapy (PDT) in patients with chronic central serous chorioretinopathy (cCSC).
Methods:
We retrospectively reviewed medical records of 68 eyes from 68 patients diagnosed with cCSC and treated with PDT. Demographic data, duration of symptoms before treatment, baseline best-corrected visual acuity (BCVA), and multimodal imaging features (SD-OCT, FFA, ICGA) were analyzed. Patients were followed up to 24 months post-PDT for recurrence.
Results:
The mean age was 48.1 ± 10.4 years; 76.5% were male. Recurrence was observed in 19 patients (27.9%). Longer symptom duration before PDT (20.9 vs 13.3 months, p = 0.001), presence of non-shallow PED on SD-OCT (68.4% vs 38.8%, p = 0.02), and RPE changes on fundoscopy (94.7% vs 71.4%, p = 0.03) were significantly associated with recurrence. No significant correlation was found with FFA or ICGA findings.
Conclusion:
Longer symptom duration and certain SD-OCT and fundus features may predict recurrence after PDT in cCSC. Recognizing these predictors could help guide earlier treatment decisions.
