Background:
Optimal sleep is essential for children's brain maturation. However, the relationship between early sleep patterns and later cognition remains unclear.
Objectives:
To examine associations between early sleep parameters and subsequent cognitive outcomes.
Methods:
This longitudinal cohort study followed 265, 267, 280, 274, and 259 children at ages 1, 2, 3, 4, and 6 years, respectively. Caregivers reported children's naptime duration and sleep ratio (nighttime sleep divided by total daily sleep) annually from ages 1-4. Cognitive development was assessed at age 4 using the Mullen Scales of Early Learning and at age 6 using the Wechsler Abbreviated Scale of Intelligence. Path analysis examined direct and indirect associations.
Results:
Children with shorter naptime durations (≤75th percentile) or higher sleep ratios (>75th percentile) at least once between ages 1-3 had higher overall cognitive and subscale scores at ages 4 and 6 than those consistently longer naptimes (>75th percentile) or consistently lower sleep ratios (≤75th percentile). Shorter naptimes and higher sleep ratios from ages 1-3 were directly associated with cognitive outcomes at age 4 and indirectly associated with full-scale IQ at age 6 through cognition at age 4. Paternal education and family income were indirectly associated with higher IQ at age 6 through pathways involving shorter naptimes, higher sleep ratios, positive parenting, and early cognition.
Conclusion:
Shorter daytime sleep and a greater proportion of nighttime sleep in early childhood were associated with better cognition at ages 4 and 6. These findings highlight the importance of supporting healthy sleep patterns during routine pediatric care.
