Medical cannabis and the hype cycle: Clinical evidence and future directions

Medical cannabis has moved from a taboo subject to a mainstream clinical discussion over the past three decades. This review synthesizes the evidence and frames its progression using a Hype Cycle lens, a framework that describes the journey of a new technology from initial enthusiasm to realistic application. Public interest and research output surged in the mid to late-2010s, yet a closer look at the evaluative literature shows that measured clinical benefits are generally modest, vary by product, dose, and population, and are limited by research challenges such as heterogeneity and blinding. Viewed through this lens, medical cannabis appears to have passed the Peak of Inflated Expectations and is progressing toward a more durable Plateau of Productivity. This new phase is marked by selective use, realistic therapeutic goals, and systematic attention to safety. Looking ahead, future progress will depend on key factors: standardized products, rigorous pragmatic trials and large registries to capture real-world effectiveness, and head-to-head comparisons with established modern therapies. Clinician education and shared decision-making can help align patient expectations with the available evidence, while pharmacovigilance and guidance for vulnerable populations can mitigate risks. In sum, medical cannabis may have a role for select patients, but its optimal use will depend on clearer evidence, standardized products, and more robust comparative studies.
