Summary

Small cell carcinoma of the urinary bladder (SCCB) is rare and aggressive. Genomic profiling identified high tumor mutational burden, predicting response to immunotherapy in this case.

Background:

  • Small cell carcinoma of the urinary bladder (SCCB) is an uncommon and aggressive cancer.
  • It presents with limited therapeutic strategies and a generally poor prognosis.

Purpose of the Study:

  • To present a case of SCCB with mixed histology and bone metastases.
  • To evaluate the utility of genomic profiling and tumor mutational burden (TMB) in guiding treatment decisions.
  • To highlight the potential role of immune checkpoint inhibitors in SCCB.

Main Methods:

  • A 63-year-old male patient with initial diagnosis of urothelial carcinoma with sarcomatoid features.
  • Bone biopsy confirmed small cell carcinoma; retrospective review showed mixed histology.
  • Genomic profiling revealed high tumor mutational burden (22 mutations/megabase).
  • Treatment included cisplatin, etoposide, followed by pembrolizumab monotherapy.

Main Results:

  • The patient achieved a complete response to pembrolizumab lasting over a year.
  • Recurrence occurred with lymph node and bone metastases.
  • High TMB correlated with initial response to immunotherapy.

Conclusions:

  • Genomic profiling, specifically TMB, can inform clinical decision-making for SCCB treatment.
  • Pembrolizumab showed efficacy in this high TMB SCCB case.
  • There is a critical need for developing novel therapeutic approaches for SCCB.
