Background:

  • Osteochondroma is a common benign bone tumor.
  • Secondary chondrosarcoma can arise from osteochondroma.
  • Increased cartilage cap thickness (CCT) is a known indicator of secondary chondrosarcoma.

Purpose of the Study:

  • To investigate the association between CCT and histologic grade in secondary chondrosarcoma.
  • To determine if CCT correlates with oncologic outcomes after primary resection of secondary chondrosarcoma.

Main Methods:

  • Retrospective analysis of 45 patients with biopsy-proven secondary chondrosarcoma.
  • Measurement of CCT from imaging or gross specimens.
  • Statistical analysis using logistic regression and survival models.

Main Results:

  • Mean CCT was significantly greater in high-grade (2 or 3) than low-grade (1) secondary chondrosarcoma (76 mm vs. 35 mm).
  • Thicker CCT was associated with increased odds of high histologic grade (OR, 1.11; P < 0.001).
  • No significant association was found between CCT and overall survival, local recurrence, distant recurrence, or margin positivity.

Conclusions:

  • CCT is a valuable diagnostic marker for identifying high-grade secondary chondrosarcoma.
  • CCT thickness is not a reliable prognostic factor for long-term oncologic outcomes.
  • The association between CCT and tumor grade has implications for surgical planning in secondary chondrosarcoma.
