Imaging Studies for Cardiovascular System VI: Calcium -Scoring CT

370
Calcium-Scoring CT ScanA calcium-scoring CT scan, also known as coronary artery calcium (CAC) scan, detects calcium deposits in the coronary arteries. This test assesses the risk of coronary artery disease (CAD), which can lead to cardiovascular events such as angina, heart failure, and sudden cardiac arrest.A calcium-scoring CT scan is generally recommended for individuals at intermediate risk of CAD without symptoms. It includes:Men aged 40-75 and women aged 50-75: Especially those with a...

Atherosclerosis II: Clinical Manifestations and Diagnostic Tests

434
Atherosclerosis is a progressive disorder that leads to the thickening and narrowing of arterial walls due to plaque buildup. This condition can cause various symptoms depending on the arteries affected:Coronary Artery Disease (CAD): This condition affects the coronary arteries and may lead to chest pain (angina), shortness of breath (dyspnea), heart attacks, and other heart disease symptoms.Cerebrovascular Disease: This affects blood flow to the brain, causing transient ischemic attacks (TIAs)...

Area of Science:

  • Vascular Medicine
  • Medical Imaging
  • Biostatistics

Background:

  • Carotid vulnerable plaques (VPs) are a significant risk factor for stroke.
  • Accurate prediction of VPs is crucial for timely intervention.
  • Current prediction methods may lack comprehensive integration of diverse risk factors.

Purpose of the Study:

  • To develop and validate a multimodal nomogram for predicting carotid VPs.
  • To assess the nomogram's precision in identifying stroke-associated high-risk plaques.
  • To support personalized clinical interventions for patients with VPs.

Main Methods:

  • Prospective study of 146 patients undergoing carotid endarterectomy.
  • Utilized least absolute shrinkage and selection operator (LASSO) regression for variable selection.
  • Nomogram performance evaluated using AUC, calibration curves, DCA, and CIC.

Main Results:

  • Identified 8 predictors: NLR, IPN grade, smoking, neutrophil count, BMI, plaque echogenicity, CRP, and morphology.
  • Nomogram achieved high discriminative capacity (AUC 0.911 training, 0.889 validation).
  • Demonstrated good calibration and favorable clinical utility.

Conclusions:

  • The developed multimodal nomogram accurately predicts carotid VPs.
  • Facilitates risk stratification for patients with carotid plaques.
  • Supports targeted preventive therapies for high-risk individuals.
