  • Neurofibrillary tangle pathology (NFTp) in Alzheimer's disease (AD) correlates with neurodegeneration measurable by MRI and FDG-PET.
  • Few studies have directly compared MRI and FDG-PET sensitivities against post-mortem NFTp assessments.
  • This study investigates the relative sensitivity of FDG-PET hypometabolism and MRI gray matter (GM) atrophy to varying NFTp stages.

Purpose of the Study:

  • To compare the sensitivity of FDG-PET and MRI in detecting neurodegeneration associated with different stages of neurofibrillary tangle pathology (NFTp).
  • To assess whether FDG-PET hypometabolism or MRI GM atrophy better reflects early versus advanced NFTp.

Main Methods:

  • Studied 88 individuals from the Alzheimer's Disease Neuroimaging Initiative (ADNI) autopsy cohort with pre-mortem FDG-PET and structural MRI scans.
  • Correlated Braak NFTp stages (post-mortem) with regional FDG-PET Standardized Uptake Value Ratios (SUVRs) and GM volumes using Spearman correlation.
  • Performed pairwise comparisons of imaging markers across grouped Braak stages (0/I, II-IV, V-VI) for significant associations.

Main Results:

  • Higher Braak stages correlated significantly with reduced FDG-PET SUVR in tempero-parietal regions typical of AD.
  • Pronounced hypometabolism was observed in early Braak stages II-IV, with further increases in advanced stages (V-VI).
  • MRI showed similar regional associations with Braak stages, but with lower effect sizes and significance only in advanced stages (V-VI).

Conclusions:

  • FDG-PET-measured glucose hypometabolism is a sensitive neuroimaging marker for neurodegeneration accompanying progressive NFTp in AD.
  • FDG-PET hypometabolism detects early NFTp-related neurodegenerative changes that appear to precede macrostructural GM atrophy measured by MRI.
