Background:

  • Social isolation and loneliness are risk factors for dementia and depression.
  • Depression is linked to cognitive decline and chronic inflammation, which accelerates brain aging.
  • Few studies have longitudinally examined how psychological factors mediate the relationship between systemic inflammation and brain atrophy.

Purpose of the Study:

  • To investigate the longitudinal impact of depression and loneliness on brain age.
  • To examine the role of the inflammatory pathway in this relationship.

Main Methods:

  • Thirty healthy older adults (mean age 72.66) from the KSHAP study underwent assessments 4 years apart.
  • Measured tumor necrosis factor-alpha (TNF-α), depression, and loneliness.
  • Used machine learning for brain age estimation from MRI and linear mixed-effects models.

Main Results:

  • Brain age increased significantly over 4 years; depression showed a decreasing trend.
  • Higher depression levels were associated with a greater increase in brain age.
  • Systemic inflammation (TNF-α) significantly contributed to brain aging, particularly in individuals with higher depression.

Conclusions:

  • Depression, not loneliness, negatively impacts brain aging through inflammaging over four years.
  • Systemic inflammation accelerates brain aging primarily in older adults with significant depressive symptoms.
  • Alleviating depression is a key preventive strategy for cognitive decline and dementia in older adults.
