Background:
Compared with angiography, there is limited evidence supporting the use of optical coherence tomography (OCT) guidance during percutaneous coronary intervention (PCI) in patients with acute myocardial infarction (AMI).
Aims:
The study aimed to evaluate the role of OCT in guiding reperfusion strategies and improving the prognosis of AMI patients.
Methods And Results:
This was an observational cohort study of 3250 AMI patients who underwent PCI between September 1, 2016, and December 21, 2019 (1901 in the OCT group and 1349 in the non-OCT group). After adjusting for imbalanced variables in the groups, OCT group was only correlated with a markedly lower risk of HF (hazard ratio, 0.78; 95% CI, 0.63 to 0.98; p = 0.029).
Conclusion:
Compared with angiography alone, OCT guidance was associated with a lower incidence of HF in AMI patients at the 1-year follow-up.
